SLS helps adults live in their own homes with the level of support they need to stay safe, stable, and connected. Through Self Determination, families can build a plan that matches the person's life, routines, and goals.

Available statewide through the Self Determination Program. Support levels and hours are based on the person's assessed needs and approved spending plan.

Always aligned with the IPP, person centered plan, and Self Determination spending plan.

WHO IS IT FOR

What SLS is made for.

SLS is ongoing support for adults who want to live in their own home safely with independence, dignity, and stability. Services are flexible, personalized, and centered on the person's choices while keeping health routines, responsibilities, and safety organized.

TITLE 17 ALIGNED

SLS focuses on health, safety, daily living, and household stability. It does not replace clinical care or become 24 hour supervision unless specifically approved in the plan.

PERSON CENTERED

Every support plan is built around the person's voice, choices, and goals. Dignity and independence are not negotiable.

SDP ONLY

WAYS offers SLS style coaching through the Self Determination Program only. Services are funded through the participant's individual SDP budget.

IMPORTANT BOUNDARIES

What SLS is not.

CFS with WAYS is available through RCEB and GGRC for adults who live with

family in the following areas.

NOT CLINICAL CARE
  • No medical procedures
  • No clinical therapy
  • No behavioral treatment plans
NOT 24 HOUR SUPERVISION
  • Unless specifically approved in the plan
  • SLS adjusts based on assessed need
  • Supports independence not dependency
NOT HOUSEHOLD MANAGEMENT
  • No deep cleaning for the person
  • No chores done without the person
  • No tasks outside the approved plan

SLS is a match when the person

WAYS SLS is designed for adults who:

  • Are 18 or older and served by a Regional Center
  • Want to live on their own safely and independently
  • Need support with home, health, and daily stability
  • Are enrolled in or transitioning to the Self Determination Program

How to start SLS through SDP

Getting started is simple and fully supported by our team.

Talk with your independent facilitator to include SLS with WAYS in your person centered plan.

Work with your FMS to add WAYS as your SLS provider and confirm approved hours.

Submit a referral to WAYS with approved hours and the person centered plan details.

Begin weekly support matched to your routines, home, and personal goals.
